The CareSimple Patient App provides a convenient method for individuals to monitor their health conditions and communication with their care team from the comfort of their own homes. This application is designed to be utilized in conjunction with a healthcare professional who will analyze the data to make informed medical decisions.

It is important to note that this application is intended for use exclusively in the United States and Canada, as it is classified as a class 1 medical device regulated by the FDA and Health Canada, respectively.

HOW IT WORKS

Upon receiving an invitation from your physician to participate in a CareSimple Remote Patient Monitoring program, you will be provided with a unique access code and a device via mail. This device gathers your measurements and transmits the data directly to your medical file at your doctor's office. Subsequently, your care team will consistently monitor your health status based on the regular readings you submit, enabling them to tailor the best care for you and your condition.

The CareSimple Patient App serves as an essential tool throughout the program. By utilizing the access code issued by your healthcare provider, you can set up an account to benefit from the following features:

CARE PLAN

Your healthcare professional will develop a personalized care plan encompassing specific instructions such as daily measurements (e.g., blood pressure checks), periodic questionnaires (e.g., monthly wellness assessments), or other necessary tasks (e.g., weekly foot checks). The app will prompt you via push notifications and display active "Tasks" on the home screen to ensure timely actions. Execution of these tasks typically requires no more than a minute.

RESULTS

All data collected through the app is accessible to your healthcare provider. Additionally, you can view this information in the "Results" section, which utilizes graphs and color codes to enhance data interpretation.

MESSAGING WITH YOUR PROVIDER

The app facilitates communication between you and your healthcare provider through text or photo messages via the "Inbox" section.

N.B. CareSimple Patient operates on an invitation-only basis. To participate, kindly inquire with your employer, health plan, or healthcare provider regarding program sponsorship.
